Harmony Health Medical Clinic and Family Resource Center is a Federally Qualified Health Center (FQHC). FQHCs include all organizations receiving grants under Section 330 of the Public Health Service Act. FQHCs qualify for enhanced reimbursement from Medicare and Medicaid, as well as receive other benefits. FQHCs must serve an underserved area or population, offer a sliding fee scale, provide comprehensive services, have an ongoing quality assurance program, and have a consumer led governing board of directors.

Team-Based Care: Patients have the opportunity to choose their primary care clinician. Additionally, a dedicated team of support staff will collaborate with them to address all healthcare needs. Harmony Health will provide access to community resources, self-management tools, and personalized care plans designed to optimize and sustain health.
Improved Tracking and Care Coordination: After a discharge from the hospital or emergency room, our healthcare team will promptly schedule a follow-up appointment. This appointment serves to monitor recovery progress and helps to prevent any unnecessary hospital readmissions. If needed, we'll identify, recommend and refer to appropriate specialists for care. Patients can expect to receive test and lab results in a timely manner. Depending on these results, we may schedule an additional appointment to ensure full understanding of outcomes and any necessary follow-up steps.
Our team will coordinate referrals to medical specialists and other healthcare services as required to optimize overall health and well-being. We are committed to providing comprehensive care tailored to specific needs.
Improved Health Access and Communication: For urgent health concerns during business hours, we have same-day appointments available with a primary care clinician. Simply call our main office number to schedule. After hours, our answering service will handle your call and connect patients to an on-call provider. In case of a true emergency, always call 911 immediately. Remember, our medical health team can address many immediate health issues effectively. By contacting us first, we may be able to avoid a lengthy and costly visit to the emergency room. We're here to provide prompt, efficient care tailored to your needs, whether during or after office hours.
Improved Communication and Access to Information: Reach out to a member of our Medical Home Team at any time during regular business hours. Our web-based system enables access to information about specific medical conditions and receive electronic reminders for appointments. We will provide additional details on how to enroll in our patient portal, MyChart.
How will I contact my Medical Home Team? To schedule an appointment, simply call Harmony Health. For after-hours care, an on-call provider will respond to your calls. However, for any emergencies, always dial 911 immediately.
It’s crucial that to inform the Medical Home Team if care is received from any other healthcare provider. This information allows us to effectively coordinate all aspects of healthcare, ensuring continuity and comprehensive management of medical needs.
